issued by the state that you are located, and send us a copy, either by fax or email.
Once you have been approved as a Gold Club member you will be able to purchase from
de-notes.com at a special Gold Club price. Once you have added your items to
your shopping cart proceed to check out and use the word GOLD in the coupon code to
receive the proper pricing. Your discount will be taken off the total order, not each item.
